  If the child has not been baptized, a copy of the child's birth certificate is needed.
- This part may be completed later. However, please note that a confirmation certificate needs to be provided by the Godparent/Sponsor. Allow plenty of time to get a copy of the certificate if the Godparent/Sponsor does not have a copy in their possession. The exception would be if the Godparent/Sponsor was baptized here at St. Joseph in Otis Orchards.
  
  Each Candidate or Catechumen must have a Sponsor or Godparent.
  
  A godparent and/or sponsor must be at least 16 years old, baptized, confirmed and a practicing Catholic. Parents cannot serve as godparent/sponsors. One godparent/sponsor is required for Baptism, sponsor. If Catholic, the Church prefers that the child's godparent be his/her sponsor. A sponsor is required for Confirmation.
  
  Catholic Godparent/Sponsors need to provide a copy of their confirmation certificate or other verification of having received the sacrament.
